YouthCare HealthChoice Change In Prior Authorization Number

Thank you for being a valued provider in YouthCare HealthChoice Illinois’ (YouthCare) Provider Network. Effective February 1, 2022, YouthCare will transition its Pharmacy Benefit Manager (PBM) from Envolve Pharmacy Solutions to CVS Caremark.

As part of this transition, the pharmacy prior authorization fax number will change. Please update your fax system and prior authorization system to reflect this change. The new pharmacy prior authorization fax number is 1- 844-205-3384.

The pharmacy benefits for YouthCare members will remain the same.

There will be no change to the Illinois Department of Children and Family Services consent fax number for psychotropic medications.
